Have you tried a google search? You might be able to find a web page or Word doc that is close to the template you'd like. I have found that it works best to design templates in MS Word using tables, then copy/paste the table into an EN Note. You have much more control over table layout/format in Word. Link to comment

As to your suggestion for prototyping something, I have looked into creating something, but Evernote really does not come with [blank] templates to be modified.
