Formatting issues and support tickets

I have two problems I need help with, I'd appreciate any direction from the community.

1. I've been having issues with the formatting of the text in Evernote for Mac. When I try to use the "code block" formatting option or convert to plain text, nothing happens. I've tried everything I can think of, and can't seem to get it to work at all.
2. I went on the website to search for help, and the support page gives you a series of drop downs to identify your problem. When I finish, it says I should submit a ticket, but there is no where to do so. I've looked all over the site, the contact page, the support page, I can't find anywhere to submit a ticket. 

If anyone knows what I could do for next steps, I'd appreciate it.
