No plug-ins are available.

Your best bet is to create a Note "template", using a table created in a word processor like Word.

In Word you can create a very nicely formatted table styled just as you like it, including title, "field names", etc.

Copy this table (not the file) into a Note that will be your template.

When you're ready to add a new item (model), just right-click on the Note template and select "Copy to..."

Then you can fill in the "fields" on the table in the Note.
