Pre-filled lists in template

I’m a field researcher who needs to log observations in bulk.  Each observation has several fields that get populated with a choice from several options - for example, a flower color might be red, blue or yellow.  I also need to attach photos to each observation.  Is it possible to set up a template with drag down menus or something similar with pre-populated answers?  Thanks!

Hi.  It's not possible to have multiple multiple-choice options in a template.  Something like Jotform or Google Docs might help.
