Mac Desktop PDF's rotate is greyed out.

Same on my Mac, tried it with several pdfs, both one-paged and multi-paged. It even shows the options as symbols at the top left of the annotation window, but this disappears once the pdf is loaded.

To me it looks as if the functions is provisioned, but not yet available. We had similar cases earlier in v10 when the GUI already showed options that were not functional at that moment. They got updated by a later release.

To be sure, issue a support ticket.

It is very simple: Just click on the pdf, so it gets a blue outline around it. Now choose open. It will open in the MacOS Preview App, where you can perform all sort of action, including rotation. 

On closing (cmd-W) the Preview window, the changes will save back into the note.

If you have chosen another app as standard pdf viewer, it will open in that app. Then it depends on the abilities of that app to manipulate a pdf file. If not happy, switch it in MacOS (standard app) to Preview, which really is doing a superb job for pdf annotation.
