iPhoneを機種変更したらログインできなくなった

Probably you are on the Free model, and now are trying to use more than 2 devices.

If this is the situation, go to the old iPhone and log out of the app. Then close the app. Now go to your other device (there must be a second, because you have 2 registered) and withdraw access for the iPhone 12.

You should now be able to log in with the iPhone 15.

You have only 2 unsyncs per 30 days. If they are used up, you need to wait for 30 days, or subscribe (for example for a month). Then you have unlimited devices, and can sort it out.

Hint: It makes it easier to see which iPhone is which if you give each phone a different name in iOS settings, General, Info.
