Best tool for reflections / journal?

I have 10 questions that I ask myself everyday (e.g. What did I do well today/ What are the 3 things I can improve on) and I currently use Google Forms so that it can be tabulated into an Excel sheet when consolidated in the future.

Is there any proper, cross-platform (Android and Windows) app that allows me to do this offline with a nice UI to boot?

I keep my data in Evernote; a daily journal note  
Not a dedicated journal reflection UI; my "questions" are included in a journal template1238163856_ScreenShot2019-12-28at5_32_31AM.png.974d9df6d8fedb409fcb8fd80753939c.png

I export to a spreadsheet for "tabulation"    
The export is automated using applescript (Mac)

I also copy the bad/average/great icon to the note title

I don't use the template feature; just notes that I label as templates
