    I turned off the "include annotation summary," and it seems to have helps right now. It doesn't happen all the time for me so I can't confirm that this is a fix, or if it just hasn't happened again yet.

    I'm also having the same issue. It does this if I have annotated the PDF Windows or Android tablet... and it's happened a lot over the years. I've generally ignored it, but I'm about to have to work with a large amount of PDFs. I attached a picture cause it is pretty bad... not even kind of close to what I highlighted.
