If you try and add a PDF form - one that you can add data to and save - to a note then Evernote falls over and dies.
If you re-start EN and then go to the note, the PDF has been stored, but as soon as you do anything in the note - adding more text or even just scrolling - Evernote falls over and dies.
You should see crash reports in your system for this from me.
