evernote randomly inserting notes

Why is evernote randomly inserting notes?  Everyday or two a new note is automatically entered and is titled as Sample.  It is frustrating.  I am thinking a third party app is creating this note and sending it to Evernote, but I am not sure where or how.

If you haven't already log into your account from the web and go to Settings -> Connected Services to see what 3rd party apps have access.
