Frustrated Trying to Publish Kustom Note Template

I have been playing with Kustom Note trying to create some forms that I can use in Evernote.  However, I cannot figure out how to publish the form from Kustom Note to Evernote.  I can create a note with filled in data but I thought I could just get the template into Evernote to start a filling in on the go.
