What happened to the widget?

Have you installed Evernote to your SD card, or your main memory?

It's on the SD card.

This is an AndroidOS limitation - you need to have it installed to the Main Memory unfortunately to have the Widget work:

I moved it back and still no widget. Do I need to uninstall and reinstall back on the main memory?

Probably rebooting the device after moving the app back to phone should do the trick.
