Customized Journal Template

We would like to create an evolving journal template for our team, but I have the following questions:

1. It is possible to make a journal for team members to use that has new pages each week?  If so, what version of Evernote will facilitate this?
   1. Can team members either keep this electronic in Evernote or print it to fill it out?
2. Can we make each week different, similar to a drive file structure? Or is there a better way to do this?
3. Can we keep it proprietary, meaning if they leave, we can revoke access?
