I get a message that I create or delete a note

It is my first participation. Excuse my grammar. 

Every time I create or delete a note, with an image or simply the title of it, a message appears for a product satisfaction survey. 

If I answer "yes", nothing happens, if I answer "no", it shows the form, but still always see the same thing. , nothing happens, if I answer no, it shows the form, but still always see the same thing. 

I've tried clearing cache, data and even uninstalling and reinstalling and nothing at all. 

This situation is starting to make me desperate. Please help me.
