If you have a paper version you can scan,  or something you can copy,  you could set up your own template.  I'm not aware of any existing ones.

Set it up yourself.

For better formatting I would use a table to show the time slots and medication doses.

There are 2 ways of doing it:

Use the template function: Create a note, edit it to your liking, then choose "save as template" in the note information menu (3 dots top right)

Create and edit the note as before. Save it, for example in a notebook called "Templates". To use it, select it, and then duplicate it. Use the copy.
