What are the real instructions for creating a new stack?

The instructions for adding a new stack include the statement below.

"Select Add to stack from the drop-down menu, then select New stack to create a new stack or select a stack name to add to an existing stack."

except that when the drop down window appears. THERE'S NO ADD TO STACK, and therefore NO NEW STACK  I can never understand why it happens so regularly that the instructions, even from authoritative sources is so often WRONG!
